Mission
Command Health and Wellness is a community-based behavioral and mental health program that launched in 2019 to address the needs of vulnerable communities. At Command Health and Wellness we believe that every family should have the opportunity to reside in a community of their own choice as they seek and receive a range of rehabilitation services that we currently offer. Our goal is to ensure that the communities we serve take control of their healthcare and thrive while achieving their goals.

This service is paid for by your health insurance through your Managed Care Organization and Medicaid.

You were referred for services by a licensed mental health professional and have been diagnosed with a particular mental health disorder that has been approved by the Behavioral Health Administration, Medicaid and Optum Health Services.
